Google Ads · Dental and orthodontic clinic
Two years of paid search for a clinic, measured in bookings
Tooth Crew Clinic is a dental and orthodontic practice in Islamabad. Dental paid search is expensive and crowded, and the temptation is to optimise for cheap clicks. We ran the account against booked appointments instead, for nearly two continuous years.
The situation at the start
A category where a wasted click is an expensive click.
Dental search terms are among the more competitive in local healthcare, and the cost of a click reflects that. An account optimised for volume will happily spend the budget on people researching prices, comparing clinics in other cities, or looking for something the practice does not offer.
The only number that matters in this category is booked appointments, so the account had to be built around conversion tracking from the start rather than reporting on clicks and hoping.

The approach
Four workstreams,
one engine.
Each stream existed to make the others measurable. Nothing here was run in isolation.
Track the booking, not the click
Nothing else can be optimised until the conversion is defined.
- Conversion tracking configured for appointment actions
- Call and form conversions both captured
- Reporting framed around conversions rather than traffic
- Spend judged per conversion, not per click
Spend only on treatable intent
Most wasted spend in healthcare comes from the wrong searcher.
- Negative keyword lists built from real search term reports
- Research and price-shopping intent separated from booking intent
- Location targeting tightened to the practice's catchment
- Treatment-level ad groups so copy matches the search
Earn the click before paying for it
An 11.1% click-through rate is a relevance outcome, not a bidding one.
- Ad copy written per treatment rather than per clinic
- Landing pages matched to the specific treatment searched
- Extensions used to answer pre-click questions
- Underperforming variants retired continuously
Two years, not two months
Paid search compounds when it is managed continuously.
- Ongoing bid and budget adjustment across the period
- Seasonal demand accommodated rather than fought
- Campaign structure revised as treatments changed
- Account maintained through platform and policy changes
The results
Nearly two years of continuously tracked bookings.
Across the period the account delivered 849 tracked conversions from 21.1K clicks, served against 190K impressions, on Rs 2.4M of managed media — an average of roughly Rs 2,827 per booking in a category where a single implant case repays that many times over. The click-through rate of 11.1% is well above what this category typically returns, which is the clearest signal that the ads were being shown to the right searches rather than simply being shown often.
The more important outcome is continuity. This was not a burst campaign: the account ran and was actively managed for nearly two years, which is what allowed query control and ad relevance to compound rather than resetting each time a campaign was relaunched.
Conversions by period

Managed spend is the account total for the period; cost per conversion is derived from that total and the recorded conversion count.
Key takeaways
What made
the difference.
- 1Define the conversion before spending
An account without conversion tracking can only be optimised toward traffic, which in healthcare is the expensive mistake.
- 2Click-through rate is a relevance score
11.1% in a competitive dental market comes from matching the ad and landing page to the exact treatment searched.
- 3Negatives are where the savings live
The search terms you exclude protect more budget than the bids you lower.
- 4Paid search rewards continuity
Two years of sustained management beats repeated three-month bursts, because the learning is not thrown away.
